Steps for using and submitting IACUC pdf forms

The following steps will allow you to complete and submit pdf forms. Should you have any issues or questions, please contact J. Pass at jpass@princeton.edu . She can assist you in the process.

  1. Download the IACUC form to your desktop.
  2. Open Adobe Acrobat or Adobe Acrobat Reader. 
  3. Select File, Open, then navigate to the file on your desktop.
  4. When finished, save the file.
  5. To submit the form, attach the file to an email to iacuc@princeton.edu.

NOTE:  If you are still encountering problems with the form, check that you have installed the latest version of Adobe Acrobat or Adobe Acrobat Reader.
